To submit a form or an application, please print the appropriate documents below and mail them to the WA ST Autumn Leaf Festival Assn. P.O. Box 116 Leavenworth, WA 98826. A member of ALFA will contact to you to confirm that we have received your paperwork.
Please Note: WA State Autumn Leaf Festival does not accept or have any vendors during the festival.
